Almost six hours of intermittent failures (red). Detection took 14 minutes; the fix that worked (green) came after scaling out had not helped.

On 29 September 2026 Microsoft's Azure OpenAI Service, Foundry Agent Service, Foundry Models and Azure AI services failed intermittently in one region. Microsoft's Post Incident Review (tracking ID 7QL5-Z50, on the Azure status history page) says that between 10:03 and 15:58 UTC "impacted customers experienced intermittent request failures, increased latency, and/or HTTP 500 and 503 errors when submitting requests to affected models and data plane APIs hosted in this region". The region was Sweden Central.

That is 5 hours 55 minutes. Monitoring "detected elevated failure rates" at 10:17, 14 minutes in. The review gives no count of customers or requests and no cost.
